Health Educator
SAMHSA Grant

 

Qualifications: Bachelor's degree in social service, health education, public health or related field and one year of community health education experience OR high school diploma and two years in community health or comparable experience. Thorough knowledge of HIV/AIDS and STD, its mode of transmission, prevention, and influential factors in health behavior particularly as it relates to the substance abuse population. Excellent oral and written communications skills in English and Spanish. Must have worked with the target population for at least twelve months prior to hiring. Strong commitment to teamwork among staff. Must be detailed-oriented and have great organizational skills

 

Reports to: Project Manager, SAMHSA Program

 

Positions Supervised: None

Duties:

     1.      Comply with program's scopes of services, as determined by contract with funding source.

     2.      Conduct education sessions with project participants.

     3.      Conduct HIV counseling, testing and referral services for project participants.

     4.      Refer clients of the project for substance abuse treatment and other services and assist them in accessing these services.

     5.      Develop trustful and confidential relationships with all clients and provide services in a comfortable and culturally consonant setting.

     6.      Maintain client data as required by the project and provide the Data Support Specialist with data as required to comply with GPRA. Assist in the preparation of required program reports in a timely manner, including collecting and synthesizing program service statistics, production of report narrative, and compiling enclosures. Submit information related to program report in advance of deadline to Project Manager.

     7.      Work and communicate directly with staff from other agencies to assure seamless service.

     8.      Conduct quality assurance reviews with the Project Manager and other staff.

     9.      Maintain a current file on community services for which clients can be referred.

     10.  Prepare and submit Weekly Schedules and required reports in a timely manner.

     11.  Comply with agency and funding source training requirements.

     12.  Participate in agency staff meetings as well as other meetings required by funding agencies.

     13.  Maintain appropriate demeanor towards co-workers, visitors, clients, supervisors and all others.

     14.  Participate in fulfilling CARF requirements.

     15.  Adhere to and enforce agency safety procedures.  Assist in the evacuation of all program participants during a fire drill, tornado drill or other civil emergency drills according to the department evacuation plan.

     16.  Participate in improvement activities using the total quality management structure.

     17. Normal work hours are 40 hours per week. However, must maintain a flexible working profile while performing job duties.  This will include, on occasion, rearranging breaks and lunch periods to meet the needs of the participants and/or scheduled work activities
